      Difficult question! From what I;ve seen, there is significant evidence to suggest that indoor only cats have several risk factors to their health including weight problems, thyroid problems, increased cancer risk, tooth problems etc as well as several behavioural problems. There is something to be said for the protection afforded to an indoor cat given the risks out there (the road for one). But I am led to believe that at least some access outdoors is necessary, whether or not that needs to mean running free through the neighbourhood or just wandering around a garden remains to be seen!
